Warranty Request Form

All returns (warranty, repair, or for credit) require an RMA (Returned Material Authorization) number. You can complete the warranty request form to obtain an RMA number prior to returning product for repair or replacement. Warranties will be authorized in accordance with the KCI policy. If it is deemed that the part is under warranty, a KCI sales representative will contact you and provide a return authorization number and ship-to address to return the product. Products will not be accepted by KCI for return if not accompanied by a valid RMA number, which must be clearly marked on the outside of the package. Return Policy

If you need to obtain an RMA you can do so online by logging into your account here, select the order you want to return from, then select "add new RMA" and fill out the required fields. KCI will then contact you within 24 business hours regarding refund and return procedures.
